Return/Exchange Instructions:

1. Fill out the form below, then click the Print Form button to print out a copy of the completed form.
2. Include the completed form inside the package containing the return item.
3. Ship the return items and form to:

If you are unable to print out the form:
1. Fill out the form below, then click the Print Form button.
2. Select Save to PDF in the print dialog box.
3. Ship the return or exchange items back to Dove Uniforms.
4. Email the PDF to service@doveuniforms.com

* Printing the form does not submit it to Dove Uniforms. Returns/Exchanges are only processed once the printed form is received with the return items.

RETURN POLICY

Return Acceptance Policy:

Dove is happy to accept all items for return or exchange within 30 days of invoice date EXCEPT:

• Name pins
• Items with custom embroidery or patches that are not applied by Dove
• Garments that have been worn, washed, altered, or are excessively wrinkled, have dirt or makeup marks, or are otherwise not in brand new condition with all tags and packaging

Items returned beyond 30 days & up-to 60 days are subject to a 10% Restocking Fee. Garments returned to us in unsalable condition may be sent back to you at your cost. Acceptance of all returned or exchanged garments is solely at Dove's discretion.

Return Shipping: You, the customer, are responsible for the cost of return shipping. Please use a traceable shipping method and keep your tracking information until you have received either your exchange item or refund - you may also email return tracking information to: service@doveuniforms.com with your name & order number in the subject line.

Processing: Please complete the form above and enclose with return items. Returns are processed in the order they are received. Failure to provide proper documentation may result in a delay in processing until we are able to locate and contact the original sender. Please provide a current physical shipping address.
